The findings provide key insights into the needs of our youth, the rate of various health and social issues, and disparities among demographic groups. By tracking changes over time, we can better recognize emerging issues and prioritize areas of need. These results could inform the development and advocacy of policies, interventions, programming, and services at the statewide, county, and local levels.

Participating in the survey will allow students to share their perspectives to inform current understanding of health trends and improve student outcomes!

The results of this survey help education, prevention, and health partners create healthier and safer environments for all students to learn and thrive.

Why it Matters

Voluntary!

Parents may opt their child out, and students can decide whether to take part even if given permission.

Confidential!

CREEHS will work closely with schools to maintain strict confidentiality. Students will not put their names on the survey. There is no way to connect survey answers to students.

Valuable Insights!

Survey results help schools, districts, and NJ State better understand student needs and trends, informing where to focus support, programming, and resources.

By helping NJ State collect these relevant data, you can support and drive change in your school, community, and NJ at large.

Administer the Survey

Schools can administer the survey between October 2025 and June 2026.

Our team at CREEHS will work closely with your schools to facilitate a smooth process and provide technical assistance.

Notify Families

Schools are required to notify parents at least two weeks before the survey administration.

Schools will be provided with pre-written parent permission materials to distribute.

Rolling Registration—District superintendents and principals can complete the online registration form now through April 2026.

By participating, schools help ensure student voices are heard, informing efforts to create healthier, safer, and more supportive environments.

New this year, all New Jersey public schools that serve 7th- and/or 8th-grade students are invited to participate! Broader participation ensures more representative and actionable data.

Rolling registration is open through April 2026. District superintendents and principals can complete the registration form by clicking the "Register Your School Today!" button.
